Intention Path Counseling

 

I do not file insurance. If you choose to go the route of selecting me as an out of network insurance provider, then you are responsible for payment in full at each session. I am happy to provide you with an invoice for services rendered which you may then present for reimbursement to your insurance provider. Understand that your insurance provider stipulates what services they are willing to reimburse and to what amount. If for any reason your insurance provider requests more information regarding my services, I will be happy to provide it directly to you.

Fees for Service:

  • Initial Intake (required): $175

  • Therapy Session (50 minutes): $125

  • Therapy Session (30 minutes): $65

  • Comprehensive Clinical Assessment (upon request): $250+

  • All non-billable contacts (billed in increments of 15 minutes): $25/increment

  • Rate for any court proceedings (including subpoenas to testify and includes travel time to and from location of proceeding): $200/hour

Intention Path Services

Wellness Coaching is not covered by insurance.

 

Fees for Service:

  • Find Your Way Package (6 sessions): $500

  • Follow-Up Session (50 minutes): $125

  • Follow-Up Session (30 minutes): $65

  • Between-Session Contact (billed in 15-minute increments): $25/increment

Therapeutic Consultation:

$125 per hour (billed in 15-minute increments)

 

Aging Life Care Management:

$125 per hour (billed in 15-minute increments)

Insurance

If you choose to go the route of selecting me as an out-of-network insurance provider, then you are still responsible for payment in full at each session. I am happy to provide you with an invoice for services rendered, which you may then present for reimbursement to your insurance provider. Understand that your insurance provider stipulates what services they are willing to reimburse and to what amount. If for any reason your insurance provider requests more information regarding my services, I will be happy to provide it directly to you. You may want to contact your insurance company ahead of time to understand how all this works.

 

The main questions you will want to ask are:

  • Do you reimburse for out of network claims?

  • Do I have to meet a deductible first?

  • Is there any variance for Telehealth?

  • How do I file my claim?

  • How much do you reimburse for out of network providers?

 

I have included CPT codes below to help with the last question so that if they don't have a general percentage, then you can just ask about each code. For example, you can ask, "For out-of-network therapy sessions using the code 90837, how much would I expect to be reimbursed?" or another similar question.

  • Initial Intake: CPT 90791 (charge to you = $175.00)

  • Therapy Session (50 minutes): CPT 90837 (charge to you = $125.00)

  • Therapy Session (30 minutes): CPT 90832 (charge to you = $65.00)
